Closure of the College of Nursing, Nurse-Managed Health Center

August 5, 2025

The Nurse-Managed Health Center (NMHC), 987 St. Sebastian Way, Suite 1500, Augusta, GA 30912 will permanently close. Our last day of patient care will be Friday, September 12, 2025.

It has been our honor and privilege to serve as your healthcare provider.

Your health and well-being remain our highest priority. So that there is no interruption in your care, we encourage you to begin selecting a new primary care provider now or as soon as possible and to inform your insurance carrier of this change.

There are several primary care service providers at several locations throughout the Augusta area. We have included a list of options and instructions to request a copy of your medical record, to assist you.

Our team will be available until 12:00 p.m. on Friday, September 12, 2025. Please contact us at 706-721-1225 before this date if you have questions or require support as you chang to a new provider. Medication refills should be requested by Monday, September 8, 2025. For any medical emergency, call 911 or visit the nearest emergency department.

Medical Record Request

Please be assured that your medical records will remain secure and confidential. If you choose to receive primary care services outside of Wellstar MCG Health, your records can be transferred to your new provider upon your written authorization. Information on how to request or transfer your medical records is available at: https://www.wellstar.org/for-patients/request-medical-records.

You may request a copy of your medical records by:

  • Logging into the Wellstar MyChart Patient Portal
  • Calling Wellstar Medical Records at 770-810-8880
  • Authorizing your new provider to submit a request on your behalf directly to Wellstar MCG Health.

Please note:

  • There is no charge for records delivered via MyChart Patient Portal or sent directly to another provider.
  • A fee of $6.50 plus tax applies to records delivered to you via email, mail, fax, or in-person.

Augusta University Nursing Associates, Inc.

The purposes of the Nursing Faculty Practice Plan are to:

  • Further the service, education, and research missions of the university, a component of the University System of Georgia governed by the Board of Regents
  • To enable faculty to maintain clinical competence, fulfill certification requirements and share expertise with other health care providers; and,
  • To provide additional revenue streams for the College of Nursing and salary supplementation for the faculty members. Integration of clinical practice, education and research through faculty practice promotes high quality, cost effective health care and increases access to care.


The Plan may also engage in charitable activities to benefit the AU College of Nursing in line with the appropriate Internal Revenue Code for charitable organizations. Clinical placement and education of students is also optimized through faculty practice. Faculty practice opportunities are helpful for the recruitment and retention of top quality faculty.
