Immunizations

USG Board of Regents has policies in place that require students attending classes at any of Georgia’s public institutions, including Augusta University, to meet established immunization requirements.

All students must provide appropriate documentation that they have fulfilled immunization requirements prior to the start of their school semester. Standard processing time for all submitted documentation is up to 7 business days.

Students Living in University Housing

All students living in on-campus housing (Oak Hall, Elm Hall, and University Village) must provide proof of Meningitis vaccinations prior to move-in day. This includes two (2) doses of Meningococcal Vaccine (MCV4, Menactra, Menveo) and preferably three (3) doses of Meningitis B Vaccine (Bexsero or Trumenba).

Please note: Students who decline to receive the vaccinations are required to sign an Immunization waiver of liability. Students under 18 years of age must have their parent or guardian sign.

Online Students

If you are an “online only” student and the program that you are in offers the waiver, you can complete the online only waiver via DocuSign. Please note that a new “online only” waiver is required to be filled out each semester. If at any time, your classes change to become hybrid or require on campus events/sessions, the immunization waiver is rescinded and immunizations must become compliant by AU and USG requirements.

You are required to fill out an immunization form based on the academic program you are enrolled in.

Non-clinical

  • Undergraduate student
  • Cyber Security student
  • Allied Health Sciences - BSHS (Bachelor of Science in Health Services) or Health Information Administration student

Graduate Students should look up their degree program to determine the correct Immunization form they need to fill out. Review the list to determine which Graduate Immunization Forms you need.

Waiving Immunizations

If you are waiving immunizations/vaccinations, go to the Immunization Waivers page and fill out the forms, please note, some restrictions apply.

Submitting Immunization Documentation

  1. Find the appropriate Immunization Form (above) for the program you are enrolled in and print ALL pages. The immunization form must be filled out, completed, and signed and dated by a Healthcare Provider. All documentation must be in English.
  2. Log into the OSH Website using your AU Net ID and PW.
  3. Submit all records and supporting documentation.
  • Scan your documents (immunization form, TB screening/test results, and lab reports) to your computer and save them to a file. Please make sure your full name, date of birth, and student ID number are on all documents.
  • Click on “Upload” on the right side of the menu. Then click on “Choose Document” and select “Immunization Records”. Then click on “Browse” and select the documents from your computer to be uploaded. Then click on “Upload”. Once the documents are uploaded, open the document to confirm that all pages uploaded and are legible.
  • Your records will be reviewed and verified by the Immunizations Department within 7 business days. Watch your Augusta University email for important notifications from the Immunizations Department. We will send you emails as your immunization status changes or if we need to communicate with you regarding your submitted documentation. Please allow us 7 business days to review your documents and send you an updated email.

Please keep in mind, that holds will be removed 24-48 hours after documents are processed.  Documents are processed in the order in which they are received.

Immunization Waivers

Students may choose to fill out an immunization waiver for religious beliefs or for other medical reasons. Students who are 100% online, and do not need to be on campus for their classes, can complete an “online only” immunization waiver.

If you have questions about whether you qualify for an immunization waiver, please email the Immunization Department at immunizations@augusta.edu.
