Use the following guidelines to submit a service request to the Division of Facilities Services, this is not where you make requests for the health system. Please make sure to complete the form so that university facilities can process it without delay.
How to Submit a Request
Key Request
Please read the important information before submitting a key request. Failure to
do so may slow down the fulfillment of your request. The requestor needs to download
and fill out the Key Request Form and email it to the email address listed below.
Maintenance Request
The requestor should fill out the online form for maintenance service requests. You
can also call or email us directly with a work order.
Project Request
A university project request is required for renovations (not routine maintenance)
that is billable to the department. The requestor should fill out the Project Request
Form, obtain all signatures, and email the form to us. An EVP signature is unnecessary
for projects that will likely cost less than $5,000.
Space Request
This is required when your department wishes to change a space's function, physical
size, and/or location. Requestor should fill out the Space Request Form, obtain all
signatures, and submit it via email.
