Clinical Experience Block 11

The PCL Clinical Experience includes: Telemedicine Patient Encounter Session, Clinical Procedures Session, and Ultrasound Session.

1:00-2:30 PM

Telemedicine Encounters (1:00 or 1:45 start) 

  • M1 students will be conducting the visit as M2 students observe
  • Complete the SOAP Presentation Debrief Qualtrics form with a faculty member during your debrief session

Clinical Procedures (1:00 or 1:50 start) 

  • M2 Students will be reviewing vital sign assessment with their M1 mentees
  • Fill out the provided vitals sheet while discussing correct methods, possible errors, and masked vs white coat hypertension 

2:40-3:15 PM

M1 Ultrasound Time 

  • M1 students will be scanning the shoulder and the brachial artery & vein
  • Students must send themselves a video of brachial vein compression with their name on it to submit to D2L for their post quiz
  • M2 students should complete the Block 2 Image Debrief Qualtrics Form while reviewing images with their mentee

3:15-4:00 PM

M2 Ultrasound Time 

  • M2 students will be obtaining the 2nd half of the RUSH exam (FAST + AAA) Upload transverse view of Aorta with measurement for post-quiz submission. 
  • Save loops of your images to review with a faculty member. You do not need to label any structures! 
  • Fill out the RUSH Block 11 Image Debrief Qualtrics form with faculty as you review your images

Telemedicine Patient Encounters

REFRESH

  • Review Medical Student Note Writing (Updated Dec 15)

  • Review Note Template Documentation Video (recommend playing at 1.5-2X speed)
  • Review PCL Note Template with minor additions to assist with chronic disease/ primary care note documentation
  • Review Example Note
  • Optional Review Block 2 Mentee EMR and MSK telehealth videos as you will be instructing them
    • Preparing patient for exam
    • Spine exam
    • Knee exam

PREWORK REQUIRED

    • Review your patient's medical history and prepare for your telemedicine patient encounter    
      • Please send your M1 a [secure] email with pertinent patient information and guidance on your recommendation for what they should cover in the visit.  Your M1 does not have EMR access currently and this is the only way they can review the patient information.    
    • Be prepared to assist your M1 setting up a telemedicine visit and documenting in the correct place and with the correct template in the EMR (videos above) 
    • Be prepared to present objective data and the assessment and plan on the day of the encounter   
    • Recorders are the only ones that write a note and will receive the majority of that note already written from their mentee in a [secure] email. 
    • Watch the SOAP presentation video and use it to assist with educating your M1s on presenting prior to and during the debrief.

POSTWORK

    • Recorders: Edit and complete your mentee’s note on your patient by 5:00 PM 48 hours after your patient encounter and sign/submit your note to Clinical Experience Preceptor for signature.  Submit to all others as a reviewer: PCL preceptor, primary care physician, and partner.  If have SP encounter, send note the PCE preceptor, PCL preceptor, partner, and mentees.  Your final is sufficient for feedback to your mentees, however you can offer additional constructive feedback as you feel is appropriate.   
    • Once your note is sent to your Clinical Experience Preceptor, fill out Staff Documentation Qualtrics Survey to receive documentation feedback.
    • Send feedback received from the Qualtrics survey to your Mentee(s) to review. If you had a SP encounter include your edited note in the email for them to review.
    • Complete the Class of 2027 PCL Clinical Experience Survey. 
    • If any of the above is not performed you may lose professionalism points.

PREWORK  

    • Review 2017 ACC/AHA HTN Guidelines (pages e24 - e27)
    • Will be instructing mentees on correct methods for taking vital signs, possible inaccuracies (ex. cuff sizes), and answering questions about masked/white coat hypertension.

Ultrasound

PREWORK

  • Review Shoulder Ultrasound Assignment Overview for Block 2 and includes example of CONSENT for an educational ultrasound. You will be walking your M1 through CONSENTING you for their shoulder images.
  • Review Dr. Rich Gordon- In depth shoulder anatomy by ultrasound
  • Review Shoulder Ultrasound Overview Example Images 
    • Complete ultrasound debrief with M1. 
    • Review workflow of uploading image on tablet and sending it to yourself via your email (last page on example image PowerPoint)
    • Your mentee will need to send one of their images by email to themselves so they can upload the image with labels to their Block 2 PCL Clinical Experience Post-Lab Quiz.
  • Review Lifesaving Ultrasound for the Undifferentiated Hypotensive Patient: Dr. Richard Gordon
  • Review Block 11 RUSH Examples
    • For Block 11, you will be practicing the SECOND half of the RUSH exam including: RUQ, LUQ, Pelvis, Aorta Transverse with measurement, and Gallbladder Longitudinal.   You will have time to practice the first half as well. You will obtain bedside/realtime feedback as you did in Block 10. You will also be assisting Class of 2027 students who will be scanning the upper extremity.
  • Gowning Video
